Not an official form from the American Heart Association. Educational only, not a diagnosis. A reading higher than 180 systolic or higher than 120 diastolic needs emergency guidance, not the next empty row.
What goes in each box
Each day has a date, then systolic, diastolic, and pulse for the morning, and the same three numbers for the evening. Pulse only if the monitor shows it. The note is for something that would explain a strange number — coffee first, stairs, a different arm — not for a guess about what the number means.
If you take two readings a minute apart, pick one rule and keep it for the whole sheet: write both in the note, or write their average in the boxes. Switching rules halfway through makes the page hard to read at the visit.
Why the sheet is two weeks
About fourteen days of morning and evening readings is a common request before a follow-up. One intense day does not describe the month. Measure the same way each time: seated, same arm, cuff on bare skin. If a clinician gave you a different schedule, follow that and use the note on day one to say so.
Paper, and when paper runs out
Print the blank sheet and keep it where the cuff lives. A file in your downloads folder is not a log until the numbers are on it. This page does not store anything. When the tab closes, the numbers are gone, which is the point of a sheet you can hold.

Questions about the printable log
Is this blood pressure log free to print?
Yes. Use the print button, or your browser’s print dialog, and choose a printer or “Save as PDF.” There is no account and no email gate.
How many days are on the sheet?
Fourteen, with morning and evening on each day. That is the two-week grid many clinics ask to see before a visit.
Should I record both readings or the average?
If you take a pair one minute apart, choose one rule and do not change it: both numbers, or the average of the pair. Write the rule in the note on day one.
Will this page tell me if my blood pressure is high?
No. It is a blank log. Category labels for a single reading are on the blood pressure chart, and a diagnosis belongs to a clinician.
